Registration Requirements

All players are required to submit an Player Contract & Physical Form before the player is allowed on the practice field.

Please download the files below, complete all required signatures, and return them as soon as possible.

Submission: Forms can be emailed to ephratayoutfootball.info@gmail.com. Please make sure to include your players first name, last name, and team when sending any paperwork via email

Scholarships

All scholarships can be applied for directly within the online registration form. If you have already registered and need assistance, please contact the board via email.

Mandatory for Practice

The following forms must be filled out and returned before a player is cleared for practices:
